CORPORATE SPONSOR INFORMATION Eligibility Any corporation interested in the purposes of the Colorado Health Information Association (CHIMA) is eligible for Corporate sponsorship. Each Corporate sponsor shall designate one (1) individual who receives the rights and privileges of Corporate sponsorship for one (1) year [as described below]. Subsidiaries, affiliates, and divisional companies are not included under the Corporate sponsorship of the parent corporation, but are eligible for Corporate sponsorship in their own right. Annual Dues Annual dues for Corporate sponsors vary according to level chosen ($300, $600 or $1000); membership is for one (1) year beginning January 1st through December 31st. All applications received after January 1st will not be prorated. All benefits must be used during the 12 month period after purchase. Any unused benefits will be forfeited. How To Apply Complete the enclosed application; the application must be signed by both a chief corporate officer and the corporate representative to whom CHIMA assigns the rights and privileges of sponsorship. Make check payable to: Colorado Health Information Management Association (CHIMA), and mail with application to Attn: Ann Nowlin, CHIMA, 301 S Estates, Salina, KS 67401. Additional Information CHIMA Annual State Meeting: This meeting is usually held in May each year with approximately 150 attendees. Each Attendee and Vendor receives an Attendance Roster. CHIMA Representation on Committees Corporate sponsors are eligible to serve on the following committees: Data Quality, Education, Legislative, Long Term Care, Advocacy & Government Relations, Physician Practice and Technology. CHIMA LASER Please see enclosed sheet regarding LASER advertising rates and submission deadlines. Corporate sponsors receive a 15% discount on advertising rates. Your check must accompany your advertisement. Corporate Corner This section of the LASER is specifically designed for you to showcase any new business developments or information you would like to get to our members. Each Corporate sponsor will be given a half page once per year for an article. This article must be submitted electronically in Microsoft Word compatible format by the submission date of your choice. Please indicate “Corporate Corner” with your submission. Member Mailings Only the designated Corporate Representative will receive announcements of all educational offerings and any other mailings to CHIMA members. Recognition in CHIMA LASER Each new sponsor (Corporate Representative) will be listed in the LASER. All Corporate sponsors will be listed in each issue of the LASER. Please submit your Corporate Logo in electronic form to the Editor. Publications All Corporate sponsors designated representative will receive a copy of each edition of the LASER. Web Site Corporate sponsors will be listed and linked on the CHIMA web site. If you wish your Corporate logo and/or tagline to be on the site you must in JPEG forms via email to the Office Coordinator.
