Professional Lifetime Achievement
Candidate must be an active or recently retired member of CHIMA for at least ten years. Exemplifies the professional image of the association.
Qualifications:
- Leadership as an officer, director, delegate, project or committee chair of CHIMA
- Leadership as an officer, director, council, project, committee or task force chair of AHIMA
- Active membership in CHIMA and/or AHIMA projects committees, task forces or other similar activities
- Active participation in state, regional and/or national association workshops and/or publication of professional articles or books
- Support of academic programs through advisory board membership, teaching, and clinical supervision
- Enhancement of professional practice through research or the development of new/improved practices
- Advancement of HIM practice into nontraditional settings
- Participation in and/or recognition by other organizations
1) Provide a narrative summary of 300 words or less explaining why the nominee should be considered for this award. Include mention of:
- Professional employment
- State and national volunteer activities (past and present)
- Specific contributions to the profession such as publishing or teaching
- Other personal and professional accomplishments

Distinguished Member
Candidate must be an active or recently retired member of CHIMA for at least three years. Exemplifies the professional image of the association.
Qualifications:
- Leadership as an officer, director, delegate, project or committee chair of CHIMA
- Leadership as an officer, director, council, project, committee or task force chair of AHIMA
- Active membership in CHIMA and/or AHIMA projects committees, task forces or other similar activities
- Active participation in state, regional and/or national association workshops and/or publication of professional articles or books
- Support of academic programs through advisory board membership, teaching, and clinical supervision
- Enhancement of professional practice through research or the development of new/improved practices
- Advancement of HIM practice into nontraditional settings
- Participation in and/or recognition by other organizations

Outstanding Volunteer
Candidate must be an active or recently retired member of CHIMA for at least one year. Exemplifies the professional image of the association.
Qualifications:
- Contributes time and energy in volunteer service to the association above and beyond what is expected as a project or committee member, project or committee chair, delegate and/or past Board member.
1) Provide a narrative summary of 200 words or less explaining why the nominee should be considered for this award. Include mention of:
- Professional employment
- State volunteer activities (past and present)
- National volunteer activities (if applicable)
Rising Star
Candidate must be enrolled at the time of nomination in an approved HIA or HIT degree program, or who has graduated in the past three months; or HIM professional (RHIT or RHIA) who has been credentialed with the past five years; AND must demonstrate significant potential for future leadership, innovation, creativity, volunteerism, service and administrative capacity. Exemplifies the professional image of the association.
Qualifications:
The candidate should include at least three of the following:
- Someone who takes risks, thinks critically, demonstrates creativity and has the capability to lead others in these processes
- Has exceptional problem-solving ability with the capacity to envision the “big picture” in health information management and to make decisions accordingly
- Demonstrates excellence in the management of personnel, finances, or administrative aspects of HIM
- Demonstrates leadership in areas of information technology, adoption and implementation
- Demonstrates commitment to the HIM profession through active participation in local, state, and national AHIMA activities – especially those that promote HIM education
- Continues educational pursuits in advanced educational endeavors and in continuing education for the appropriate credential
